1. This product can only be sold for the following symptoms and causes.
Please confirm:

The eye problem is likely due to bacterial conjunctivitis, with symptoms such as:

Red, sore or inflamed eye(s)

Sticky discharge or crusting on the eyelids

There are no serious warning signs, including:

Severe pain or sensitivity to light

Blurred or reduced vision

Cloudy eye or unusual pupil shape

Rash on the face or scalp

Swelling of the eyelids

A suspected foreign object or recent injury to the eye

2. Medical history and eye health
Please confirm:

The patient has not had eye surgery or laser treatment in the last 6 months.

There is no history of glaucoma or dry eye syndrome.

The patient is not currently using other eye drops or ointments.

The patient does not wear contact lenses, or understands that lenses must not be worn during treatment or for 24 hours after.

3. Allergies and blood disorders
Please confirm:

The patient is not allergic to chloramphenicol or any ingredients in the product (e.g. boric acid, cetostearyl alcohol, or benzyl alcohol).

There is no personal or family history of blood disorders, such as aplastic anaemia or low blood cell counts.

4. Other medicines and health conditions
Please confirm:

The patient is not taking chemotherapy or immunosuppressants (e.g. azathioprine).

The patient is not pregnant or breastfeeding, or has received medical advice before use.

This is not a repeated episode of conjunctivitis without clinical assessment.

5. Safe and correct use
Please confirm:

You understand this medicine is for external eye use only, and will be used exactly as directed for a full 5-day course, even if symptoms improve sooner.

You will seek medical advice if symptoms do not improve within 2 days, or if they get worse at any time.

You understand this treatment is for short-term use only and repeated or extended use without medical advice is not recommended.
